The State of Illinois School Code requires a physical examination of all children within one year of entering Kindergarten.

You are advised to schedule your student's exam as soon as possible. Completed forms should be returned to the school's health office by the first day of school.

The health history portion must be completed by the parent or guardian and SIGNED. Please include any allergies or medical conditions.

According to state law, the physical must include the diabetes screening, a lead assessment, physical examination results, and a listing of dates immunizations were completed. We suggest you keep a copy of the physical for your personal records. By law, a student will be excluded from school if these requirements are not met by October 15th.

For families who might find these requirements a financial burden, please call the school's health office for assistance. Immunizations are available from the Kane County Health Department: call 630-264-7665 to make an appointment.

To be considered up to date with immunizations, the kindergarten student must have the following noted on the Certificate of Child Health Examination form:

• DPT - four or more doses with the last being a booster, on or After the fourth birthday.
• POLIO - four doses, with the last being a booster, on or After the fourth birthday.
• HEPATITIS B - three doses, first two doses received no less than 4 weeks apart, and third dose at least 2 months from second as well as at least 4 months from first.
• VARICELLA (chicken pox) - two doses; the first one received on or After the first birthday and the second no less than 28 days after, OR documentation of having had the disease.
• MMR - two doses with the first dose received on or After the first birthday, and the second no less than 28 days after.

Kindergarten students are also required by law to submit proof of a completed DENTAL exam and VISION exam.

If your child should require any medication during the school day, a Kaneland Medication Authorization form must be completed and signed by the parent and physician. 

Kaneland offers Early Childhood Screening for residents of the district.  Milestones that are checked include speech, hearing, vision, and motor skills. If you reside within the Kaneland School District, and suspect that your child(ren) ages 2.9 months & older have any delays in developmental milestones, you are encouraged to attend one of our quarterly screenings.

As proof of student residency,  at least one document listed in Category A or at least two documents listed in Category B must be brought to the school.

Category A – Attach a copy of at least one of the following documents:

• Signed lease for my residence showing an occupancy date on or before date of student registration.
• Signed real estate contract showing closing date.
• A closing statement for the purchase of residence showing a closing date on or before date of student registration.
• A home construction contract showing expected completion date.*
• An affidavit from the owner of my residence stating that I reside at the residence on a full -time basis.

Category B – Attach a copy of at least two of the following documents:

• Driver’s license
• Gas or electric bill
• Public Aid card
• Voter Registration card
• Home/Apartment Insurance Certificate
• Automobile registration from the State of Illinois

Who are they?

A McKinney-Vento eligible child is one who lacks a fixed, regular and adequate nighttime living situation. In general, children or youth living in motels, transitional housing, shelters, the streets, cars, abandoned buildings, and other inadequate accommodations are considered eligible for McKinney-Vento services. This includes the following groups:

• Children in Shelters
• Doubled-up Children (living with another family due to lack of a permanent residence)
• Children living in Motels due to lack of alternative adequate living situation
• Migratory Children (if accommodations are not fit for habitation)
• Runaways (children who have run away from home and live in a shelter or inadequate accommodations, even if parents are willing to provide a home)
• Lockouts (children whose parents or guardians will not permit them to live at home)
• These last two categories are UHY, child not in the physical custody of their guardian and lacking a fixed, regular and adequate place.

What are their rights?

The Education for Homeless Children and Youth program, as part of the McKinney-Vento Homeless Assistance Act ensures homeless children the right to:

• choose either to stay in the school of origin (school last attended or school attended when child lost permanent housing) or attend the school serving the area they are temporarily staying. Their Caregiver makes the choice.
• immediate enrollment even if medical or other records cannot be produced at time of enrollment
• receive assistance with transportation if needed
• priority to preschool programs for eligible youngsters
• free lunch and fee waivers
