New Student Info

The State of Illinois School Code requires a physical and immunization record for all children entering Preschool, Kindergarten, Fifth, and Ninth grade. These must be submitted to the nurse’s office by the first day of school. An oral health examination is also required for children in Kindergarten, Second and Sixth grade. Students will be excluded on October 15th, if the physical and immunization requirements are not complete. The Health History on the top of page 2 must be completed and signed by the parent / legal guardian.

Families who transfer from out of state are also required to complete a physical and all immunizations. Physical examinations must be no earlier than one year prior to Kaneland entrance date. Students entering school from out of state after October 15th have thirty days to complete the physical.

If your 7th, 8th or 9th grade child will be participating in a spring sport next year, you will need to wait until June to get their physical. The physicals are good for only one calendar year. Sports physical forms can be downloaded or obtained at the school office.

For families who might find these requirements a financial burden, there are low cost services available. Please call the nurse’s office for further information or call the Kane County Health Department at 630-264-7665.
