New Student Info

The State of Illinois School Code requires a physical and immunization record for all children entering Preschool, Kindergarten, Sixth, and Ninth grade. These must be submitted to the nurse's office by the first day of school. An oral health examination is also required for children in Kindergarten, Second, and Sixth grade.

Students will be excluded on October 15th, if the physical and immunization requirements are not complete. The Health History on the top of page 2 must be completed and signed by the parent / legal guardian.

Families who transfer from out of state are also required to complete a physical and all immunizations. Physical examinations must be no earlier than one year prior to Kaneland entrance date. Students entering school from out of state after October 15th have thirty days to complete the physical. The following are required by the state of Illinois:

  • Lead Screening is recommended as part of the health examination for children age 6 or prior to admission to a preschool.
  • Diptheria, Tetanus, Pertussis (DTP): Any child 3 years of age or older must show proof of 4 or more doses of this vaccine, no less than 4 weeks apart.
  • Polio: Has had 3 or more doses no less than 6 weeks apart.
  • Hepatitis B: The first 2 doses must have been received no less than 4 weeks apart, and the interval between the second and third must be at least 4 months. If the shot series has been started and the child enters early childhood, verification of vaccination appointments must be presented to the nurse.
  • Hemophilus Influenza Type B (HIB): Any child 2 years of age or older must receive one or more doses of HIB vaccine with the last dose administered on or after 15 months of age.
  • Measles, Mumps and Rubella: Any child 2 years of age or older must show proof of one dose of MMR not earlier than 12 months of age.
  • Chicken Pox: Beginning in the fall of 2002, must show proof of having received one dose of Varicella on or after the first birthday or show proof of the disease by the health care provider. The school nurse may accept a parent/guardian description of the disease.
  • For families who might find these requirements a financial burden, there are low cost services available.
