Sports Physical      Insurance Waiver      Consent to Random Testing

Students desiring to participate in interscholastic athletic competition must obtain a physical examination annually. Proof of the physical examination will be kept on file in the athletic office. No student will be permitted to practice or participate until proof of a physical examination is received.
