Health Requirements for the 2022-2023 School Year
All students must be up-to-date with physical examination and immunization requirements. All completed forms must be turned into the School Health Office before October 15.

Physical Forms & ImmunizationsAll students entering early childhood, kindergarten, 6th grade, and 9th grade are required to have a current physical examination completed via the approved Illinois Department of Public Health Form. These are requested prior to the first day of attendance but required by October 15th. Parents/guardians must complete & sign the Health History section on page 2 of the physical exam report.An IHSA / IESA sports physical does not fulfill the requirement.Reports may be dated no earlier than one calendar year prior to the first day of attendance.

Immunization Requirement InformationAll students must be up-to-date with the State of Illinois immunization requirements.All students entering sixth, seventh, eighth, ninth, tenth, and eleventh grade must show proof of receiving one dose of Meningococcal vaccine (meningitis).All students entering twelfth grade must show proof of receiving one dose of Meningococcal vaccine after the age of 16.All students entering grades six through twelve must show proof of receiving one dose of the Tdap vaccine.

Physical Exams & Immunization Requirements by Grade Level.

PreschoolComplete Preschool Physical Examination.
No additional immunization requirement if up-to-date.

KindergartenComplete Kindergarten Physical Examination and all required immunizations including:
 -Two doses of MMR Vaccine
 -Four doses of Polio Vaccine
 -Four doses of DPT/DPTaP Vaccine
 -Two doses of Varicella Vaccine (chicken pox)
Complete Kindergarten Dental Examination- Must be turned in prior to May 15, 2023.
Complete Kindergarten Vision Examination- Must be turned in prior to October 15, 2023.

1st Grade No additional immunization requirement if up-to-date.

2nd GradeNo additional immunization requirement if up-to-date.
Complete 2nd grade Dental Examination.- Must be turned in prior to May 15, 2023.

3rd, 4th, & 5th Grades No additional immunization requirement if up-to-date.

6th Grade Complete 6th grade Physical Examination and all required immunizations including:
 -One dose of Tdap Vaccine
 -One dose of Meningococcal Vaccine (meningitis) after the 11th birthday.
(If your child turns eleven after August 1st, you must submit proof of an upcoming scheduled appointment)
 -Two doses of Varicella Vaccine (chicken pox)
 -Three doses of Hepatitis B at the required intervals
Complete 6th grade Dental Examination- Must be turned in prior to May 15, 2023.

7th & 8th Grades No additional immunization requirement if up-to-date.

9th Grade Complete 9th grade Physical Examination
Complete 9th grade Dental Examination.- Must be turned in prior to May 15, 2023.

10th & 11th Grades No additional immunization requirement if up-to-date.

12th Grades Proof of receiving two doses of meningococcal vaccine (meningitis) withthe second dose received on or after the 16th birthday, is required. If thefirst dose was given after the age of 16, then only one dose is required. For more information, please view the Meningococcal Vaccine form.

Additional Health Information

New Students (to Illinois Schools)New students to Illinois schools need to provide documentation of (1) physical exam, (2) complete eye examination (3) dental exam from the most recent of K, 2, 6, or 9. The records are due within 30 days of entering school and will be reviewed for compliance with Illinois requirements.

Medications at SchoolOver the counter medications as well as prescription medications will only be administered in school when a District 302 Medication Authorization Form is on file in the health office and has been completed by the parent/guardian and the healthcare provider.
Medication Forms & Policies

Transfer Students (within the state of Illinois)Transfer students within the state of Illinois need to provide documentation of the most recent physical, eye, and dental reports that meet the state requirements listed in the grades above. Documents are due within 30 days of the first day of attendance.

Eye ExaminationsExam forms are requested prior to the first day of attendance but required by October 15th Exams must be conducted by a licensed optometrist or ophthalmologist or a physician licensed to perform complete eye examinations Reports may be dated no earlier than one calendar year prior to the first day of attendance
Dental ExamsDocuments are due for all K, 2, 6, & 9 grade students by May 15th of the school year Reports must be dated within 18 months prior to the May 15th deadline
